Consent statement:

All information collected is strictly confidential and will only be used to enable the health care team to provide you with quality health care services. By submitting this form you have read, you understand and you agree to the following consent statement:

•    I understand the reasons why information is collected from me;
•    I understand that I can choose not to provide any information requested of me, but my failure to do so may compromise the quality of care I receive;
•    I understand that I have a right to access the information collected about me, except in certain circumstances in which an explanation will be given to me;
Recording Notice

Please be informed that sessions may be recorded for transcription and note-taking purposes only. These recordings will be used solely to enhance the accuracy of your session notes and will be deleted immediately after transcription is completed. Your privacy and confidentiality are of utmost importance, and no recordings will be shared with anyone outside of this practice. By signing this form, you consent to the recording of your sessions under these terms.

•    I understand that if my information is used for any other purpose other than set out above, my further consent will be obtained.

Cancellation Policy: 

Please provide at least 24 hours’ notice for cancellations or rescheduling. Sessions canceled with less than 24 hours’ notice or missed without notice (‘no-show’) will be charged the full session fee. Thank you for understanding that this time is set aside just for you
